This is the checklist I wish I'd had when I onboarded my first chiropractic client in LeadConnector. It's the complete set of custom fields, pipeline stages, email sequences, SMS flows, calendars, and workflows that a chiropractic sub-account actually needs to run. Not a theoretical list — this is drawn from agencies running real chiro books at scale.
Use it as a spec whether you're hand-building, buying a template pack, or using an AI tool. If any of the items below are missing from your setup, your client's nurture is leaking leads.
Custom fields (21)
Custom fields are the bones of the entire sub-account. Everything else keys off them — pipeline automations read them, emails personalize from them, SMS branches on them.
Clinical intake:

Pipeline (6 stages)
The pipeline should mirror the real chiro sales cycle — the generic "New Lead → Qualified → Closed" shape doesn't fit because chiro has a care-plan commitment step that's do-or-die for revenue.
- Initial inquiry — lead came in from form, Google, or walk-in. Waiting for intake SMS reply.
- Exam booked — they confirmed an exam slot. Usually a $27 or $47 new-patient special.
- Exam complete — they came in. Provider has done the posture scan, X-rays, and initial adjustment.
- Care plan approved — they agreed to the recommended plan (usually 12-24 visits). This is the revenue-defining stage.
- Active care — currently attending visits per the plan.
- Maintenance / won — plan complete, shifted to 1-2 visits per month for upkeep. This is your lifetime value stage.
Email sequence (12 emails across ~180 days)
The cadence that actually works for chiro nurture:
- Day 0: Exam confirmation — what to expect at the first visit.
- Day 1: Pre-visit prep — what to bring, how to dress.
- Day 2: One-question intake — what's the #1 thing they want to fix?
- Day 4: Post-visit check + Google review request.
- Day 7: Care plan reminder — consistency is key.
- Day 14: Pain-scale check-in. Have them reply with a number.
- Day 21: Referral ask — we save 2 new-patient spots a week for referrals.
- Day 30: One-month check-in, shift to maintenance talk.
- Day 45: Patient story — someone similar who got results.
- Day 60: "We miss you" — no pressure reactivation.
- Day 90: Preventive care reminder.
- Day 180: $27 reactivation special — 14-day window.
Two notes: subject lines matter more than body here (most chiro opens are on mobile, people decide in 2 seconds), and the day-45 patient-story email converts best but requires a real story — never fabricate one.
SMS flow (6 messages)
SMS pulls 3-5x better reply rates than email for chiro clients:
- Day 0, +1 hour: "It's [provider] from [practice]. Got your booking. Reply with any questions."
- Day 1, morning: Appointment reminder. C/R reply.
- Day 7, mid-morning: Pain scale check.
- Day 30, afternoon: Monthly check-in — book link.
- Day 60, mid-morning: "Haven't seen you."
- Day 120, afternoon: $27 reactivation SMS.
Calendars (3)
- New patient exam — 60 min. Includes posture scan, care plan review, first adjustment.
- Adjustment (returning patient) — 15 min. Standard follow-up.
- Consultation (no exam) — 20 min. For insurance questions, second opinions.
Workflows (4 automations)
- New patient intake — Fires on contact created. SMS → email (5 min delay) → add to pipeline → wait 1 day → day-1 email.
- Exam reminder — Fires on appointment scheduled. Wait until 24h before appointment → SMS reminder → 2h before → SMS confirm.
- Post-exam nurture— Fires on appointment complete. Moves to "exam complete" stage → wait 2 days → day-4 email → wait 3 days → day-7 email.
- Reactivation — Fires on no activity 60 days. SMS → wait 3 days → day-60 email → wait 60 days → day-120 reactivation email.
Funnel structure
Two pages. Don't get creative here.
- Landing:hero + trust bar + services + FAQ + booking form. Hero headline should reference outcomes, not generic "chiropractic care." CTA books the $27 new-patient exam.
- Thank-you: confirmation + SMS opt-in note + what happens next.
What you can skip
Chiro-specific things that sound useful but don't move the numbers:
- Long-form blog funnels. Chiro conversion is word-of-mouth + local SEO, not content marketing.
- Multi-step quizzes. People in pain want to book fast, not take a symptom assessment.
- Heavy drip sequences beyond 180 days. Reactivation works; year-long drip annoys.
